Is It Time to Move? Next Steps in Care

At some point, most of us will need long-term care, but knowing when it’s time for additional support is not always obvious. This program will provide tools to help you decide when and how to take the next steps in care. You will also learn strategies to guide difficult conversations about transitioning to a care facility.
