Through expert training, measurable outcomes and a commitment to compassionate care, this Parkinson’s Foundation program helps senior living communities and home care agencies create environments where residents and clients with Parkinson’s can thrive.

Partners gain access to expert education, practical resources, ongoing support and national recognition. Community Partners equips staff with the confidence, skills and tools to deliver person-centered Parkinson's care through comprehensive education grounded in the latest clinical recommendations and best practices.

Parkinson's disease is a complex, progressive neurological condition and many people entering assisted living, skilled nursing or home care have advanced symptoms that require specialized knowledge.

This program is designed for staff working in:

- Skilled nursing homes
- Assisted living communities
- In-patient rehabilitation centers
- Home care and hospice agencies

Across the country, these organizations are on the front lines of Parkinson’s care — and we are here to ensure their teams feel confident, capable and supported.

### What the Program Offers

As a Community Partner, your organization becomes part of a growing national network dedicated to improving the lives of people with Parkinson's. The Parkinson's Foundation actively promotes Community Partners as trusted resources for people and families seeking knowledgeable Parkinson's care — helping connect our community with organizations that share our commitment to excellence.

Community Partners in Parkinson’s Care provides access to the Parkinson’s Foundation care staff curriculum, a proprietary training designed specifically for staff working with people living with PD.

Our program focuses on practical, day-to-day strategies that make a measurable difference:

- **Medication Timing** – understanding why timing is critical in symptom management.
- **Mobility Support** – promoting safe, effective movement strategies to reduce risk.
- **Patience &amp; Understanding** – building empathy to support residents and clients with dignity.

By the end of training, Partners are ready to meet people where they are in their Parkinson’s journey — equipped with both knowledge and compassion.

- ###   About Partnership   
    
    Education is at the heart of the Community Partners in Parkinson’s Care program. Our curriculum is based on the latest Parkinson’s information, pharmacological and non-pharmacological therapies, care recommendations and interprofessional communication.
    
    Developed by PD educators, with decades of care experiences, this program provides direct care staff and other professionals with knowledge they can apply to client care tomorrow.
    
    Parkinson’s disease is a complex neurodegenerative disease. Most clients who move to a senior residential community or bring in-home care are in the mid-late [stages of PD](/understanding-parkinsons/what-is-parkinsons/stages "Stages of Parkinson's") with challenging care needs. Providing individualized care to support client needs requires knowledge and confidence in [common symptoms](/understanding-parkinsons/movement-symptoms "Movement Symptoms") and [management tools](/living-with-parkinsons/treatment "Treatment"). The program distills the complexity of PD into applicable information and tangible tools to help staff immediately provide optimal care to enhance their clients’ quality of life.
    
    ### Virtual Training
    
    This program provides a full curriculum of Parkinson’s education through virtual trainings. Presented via online synchronous and asynchronous learning experiences, we educate site champions at each partner site utilizing the train-the-trainer model.

- ###   Partnership Requirements   
    
    **Maintain Active Site Champions**: Site champions are required to participate in a live virtual training course highlighting: tailored Parkinson’s education, medication management, mobility, communication, mid to late-stage complexities, and milestones of partnership maintenance.
    
    **Outcome Measures:** Site Champions are responsible to disseminate periodic surveys and complete reports during the year.
    
    **Staff Training:** Direct care staff must complete online asynchronous education within six months of program initiation.
    
    **Partner Calls** **with Community Partner's Team:** Private virtual calls to discuss progress, share information and to ensure partner questions are answered.
- ###   How to Apply   
    
    Below we highlight the Partnership process:
    
    
    1. Complete the online Partner application for your intended partner type linked below.
    2. The application is reviewed for acceptance.
    3. Complete invoice and payment.
    4. Once payment is received, site champions are invited to register for live virtual training and given access to online courses.
    
    After completing training, site champions return to work and share materials with staff to ensure direct care staff complete the online course.

    ### Senior Living Communities
    
    *Subsidies available upon need, availability and request*
    
    **Residential Home**  
    Small care settings are typically located in residential neighborhoods. They often operate under an assisted living license in a home-like setting and serve 5-8 residents. Corporate rates do not apply within this category.
    
    
    - **1 owner with 1-2 sites:**
        - 1-year partnership: $1,500
        - 2-year partnership (10% discount): $2,700
    - **1 owner with 3-5 sites:** 
        - 1-year partnership: $2,500
        - 2-year partnership (10% discount): $ 4,500

    **Assisted Living**  
    Housing for elderly or disabled people that provides nursing care, housekeeping and prepared meals, as needed. Residents typically live in an individual unit or apartment and receive services as outlined in their designed care plan. Some assisted living facilities offer specialized services or units designed for those needing memory care or other enhanced services. Assisted living typically does not provide 24-hour on-site nurse support and may be monitored by nursing assistants/aides through the night.
    
    
    - 1-year partnership: $2,500
    - 2-year partnership (10% discount): $4,500

    **Skilled Nursing Facilities, Long-Term Care, Inpatient Rehab Facilities, and Combined Services**  
    Often called skilled nursing facilities, long-term care or nursing homes, these offer comprehensive nursing care to residents in individual or twin room settings. These facilities are utilized by those needing higher levels of specialized nursing care on a 24-hour basis.
    
    This category also includes inpatient rehabilitation facilities that provide short-term rehabilitation and nursing care after an illness or injury (often covered by Medicare or insurance post-hospitalization).
    
    Combined services refer to life plan communities and continuing care retirement communities that offer multiple levels of care and services on one campus.
    
    
    - 1-year partnership: $2,750
    - 2-year partnership (10% discount): $4,950

    ### Home Care Agencies &amp; Hospice
    
    *Subsidies available upon need, availability and request*
    
    **Home Care Agency**  
    Home care agencies that offer interdisciplinary care within a client’s home may include nursing, rehabilitation therapies, social services or home health aide care that is eligible for Medicare or other third-party payer reimbursement. Most clients receive these shorter-term services following an illness, injury or hospitalization.
    
    Home care agencies that offer longer-term assistance with personal care, companionship, housekeeping, and other services. A supervising nurse or agency administration designs care plans. Visits from home, health aides, and companions can range from 2 to 24 hours of care in an individual’s home. These services are typically not covered by medical insurance (selectively covered by long-term care insurance).
    
    **Hospice**  
    Agencies that provide specialized care for those nearing end of life (life expectancy typically six months or less). These groups offer a variety of services, and most hospice programs are typically covered through a special benefit of Medicare or other third-party insurance. Hospice staff may see clients in a variety of settings, including at-home and those living within assisted living or long-term care settings.

    - Payment options: Credit card or ACH/e-transfer.
    - Notification of renewals are based on partner subscription date.
    - Invoice will be emailed one month prior to partnership expiration.
    - Partnership must be active to have access to educational and marketing materials.
    
    ### Cancellation Policy
    
    Partnerships can be canceled within 30 days of start date (or renewal start date) with written notice to [CommunityPartners@Parkinson.org](mailto:CommunityPartners@parkinson.org). A refund minus $150 service fee will be processed within 60 days of notice. At that time, the partner will forfeit any usage of member education or recognition including but not limited to the badge, display of partner certificate and marketing materials.
    
    There are no refunds after 30 days of start date of membership.

- ###   Role of the Site Champion   
    
    ### What is a site champion?
    
    A site champion is the designated representative between a partner and the Community Partners. They participate in update meetings and virtual site visits to review progress and answer questions.
    
    Site champions must participate in a one-day virtual training, where they will receive a high-level education on Parkinson’s disease, as well as information on the mechanics, expectations, and resources of the program.
    
    Successful site champions tend to be enthusiastic, organized and interested in improving the quality of Parkinson’s care. Many are nurses or social workers but can serve in any role within their organization.
    
    ### The Role of a Site Champion
    
    Typically, each partner has at least two site champions who are responsible for stewarding care staff to complete the online course and implement the tools learned in the everyday care of their clients.
    
    **Site champions:**
    
    
    - Are the primary representatives of the program at their site.
    - Ensure education for new and existing staff using the provided Community Partners online curriculum.
    - Raise awareness of partnership.
    - Perform timely collection of outcome data and reports based on the schedule provided.
    
    As the network continues to grow, the Parkinson’s Foundation takes pride in meeting with site champions during the year for one-on-one calls with our Community Partners team. Our goal is to coach and mentor site champions to support their efforts to meet the requirements of partnership each year.
    
    ### For New Site Champions Joining A Current Partner Site
    
    
    1. Complete online partner application.
    2. Application is reviewed for acceptance.
    3. Person(s) is/are invited to the next quarterly training.
